Is it possible to have 2 leaders or 2 followers in a r/s

Is it necessary to define the role earlier in the r/s like who is the leader / follower?
What Do you call, a person with sympathy but zero empathy?

I think it falls into place naturally? ???

There is definitely a hierarchy like in my case  - i'm the General in the household and my wifey is the Major.

I make the general decisions and she makes the major decisions .... :P :-[

Is it possible for the follower to be the big brother character.  :-[
